USAging’s Strengthening Social Connection in Communities


USAging’s Strengthening Social Connection in Communities (SSCC) is accepting applications for grants to enhance the health of older adults and their caregivers by replicating and sustaining evidence-based and evidence-informed social connection programs, partnerships and referral processes in communities. Proposed projects must replicate or expand an existing social connection program for older adults within an organization, develop or expand a program through cross-sector partnerships with community-based organizations (CBOs) and health care partners, or implementation of improved processes for referrals between health care providers and CBOs for social connection programs in the communities they serve. Eligible applicants include, and are not limited to, community-based social service organizations working with older adults, community care hubs, organizations connected to the Aging Network, and more. Applicants can request up to $35,000. Applications are due December 5, 2025. Learn more and apply.
